The form design inside UPLIFT Checker is a small lesson in Philippine civic UX. The product collects six fields — four about the citizen's name, one about birth date, and one about barangay — and it treats each of them with a level of care that reflects real Filipino naming conventions. The Last Name placeholder inside UPLIFT Checker shows ex. DELA CRUZ, modeling the compound-surname pattern that is common in the Philippines. The First Name placeholder shows ex. JUAN MIGUEL, modeling the typical two-word given-name pattern. The Middle Name and Extension Name fields are both labeled (If applicable) and are not marked as required.

That optional-handling for Middle Name and Extension Name is more thoughtful than it sounds. The Philippines uses a unique naming system where almost every citizen carries a Mother's Maiden Name as a Middle Name; many older citizens also carry Extension Names like Jr., Sr., II, or III. UPLIFT Checker treats the Middle Name as optional because not every record includes it cleanly, and treats the Extension Name as optional because it can appear in multiple formats. By accepting blank values in those slots, UPLIFT Checker avoids the false-negative trap where a legitimate citizen is reported as "not on the list" simply because a field is formatted slightly differently than the source data expects.

The required-field handling inside UPLIFT Checker is equally careful. Last Name, First Name, Birth Date, and Barangay are all marked with an asterisk and cannot be submitted blank. The submit button — Search and check — runs a frontend validation pass before sending the request, so the citizen never wastes a network round-trip on a typo. If the citizen tries to submit an incomplete form, UPLIFT Checker highlights the missing slots and refuses to proceed. That immediate feedback matters on slow mobile connections, which are common outside urban centers in Bulacan.

The Birth Date input inside UPLIFT Checker uses a native date picker rather than three separate dropdowns for day, month, and year. That choice reduces the cognitive load on elderly users who may not remember the exact format the picker expects. The result is a form that respects the diversity of the citizen population it serves.
